The clinical significance of preventive use of antibiotics in polluted wound

Abstract: OBJECTIVE The preventive use of antibiotics before operation and examine the effect of preventing infective complications. METHODS Operations of standard Altemye were applied.Before the operation preventive administration of antibiotics was used for one group of 100 patients during May 1996 and April 1998.At the same time,the antibiotics is not used in another group of the 100similar patients who were compared.The following was used in the preventive administration group:amikacin sulfate of 0.4g plus 0.5% metronidazole of 100ml through I.V.gtt in 33 of 100;cefazoline of 4g plus 0.5% metronidazole of 100ml through I.V.gtt in 32 of 100;cefatazidine of 1g though I.V.plus 0.5% metronidazole of 100ml through I.V.gtt in 45 of 100.For those whose operation was over 4 hours,antibiotics of one dose was reused during the operation.Then infected complications after the operation were observed and the 2 groups were compared in the infection of wound. RESULTS 3 patients of the preventive administration group were infected,Amikacin Sulfate&Metronidazole was used in 2 of 3 and Cefazoline&Metronidazole were used in 1 of 3.Compared to the control group ,9 patients got infected in the wound,2of which were done by Escherichia,another one of which by S.aureus. CONCLUSION Operation for polluted wound in which preventive adminisrtation of antibioties was used have given effect on preventing the complication caused by infection after operation especially the infection of wound.From chosen antibiotics,Cefatazidine would be more effective.
