The effect of triamcinolone acetonide delivered by microneedle roller on nude mouse model xenografted with human hypertrophic scar

摘要: 目的 观察滚轮微针对醋酸曲安奈德(TAC)的抗瘢痕疗效影响。 方法 将人增生性瘢痕移植于裸鼠背部建立模型。取模型裸鼠72只按随机数字表法分为单纯乳膏组、微针+乳膏组、注射液组,每组24只。给药后观察裸鼠的瘢痕生长情况,于15、30、45、60 d每组随机处死各6只,切取瘢痕组织行病理学观察,免疫组织化学法检测处理后跨膜糖蛋白(CD34)和α-肌动蛋白(α-SMA)表达,原位末端标记(TUNEL)法检测细胞凋亡。 结果 给药60 d,乳膏组大体形态无明显改变,微针+乳膏组和注射液组瘢痕缩小、变平、变软,色泽变淡。给药60 d,乳膏组瘢痕中CD34、α-SMA免疫组织化学和细胞凋亡分别为15.83±1.84、47.36±1.95和21.50±3.62,与给药15 d相比均无显著改变(P>0.05);而微针+乳膏组30 d后、注射液组15 d后即可见CD34和α-SMA表达减少、发生凋亡的细胞数增加,有显著改变;给药60 d 后两组的上述指标分别为7.83±0.75和9.87±1.96,26.30±8.48和27.23±4.68,34.00±1.55和38.67±3.98,均较乳膏组差异显著(P<0.05)。而微针+乳膏组与注射液组比较,60 d后各项指标的差异无统计学意义(P>0.05)。 结论 在人增生性瘢痕裸鼠模型中,滚轮微针提高醋酸曲安奈德的抗瘢痕疗效,与瘢痕内注射给药相当。Abstract: Objective To investigate the effect of triamcinolone acetonide cream (TAC) delivered by microneedle roller on the treatment for human hypertrophic scar. Methods The model was established by xenografting human hypertrophic scar onto the back of nude mouse.Seventy-two nude mice were randomly divided into TAC itself,TAC with microneedle and intralesional injection group with 24 mice in each group.Six nude mice from each group were euthanized at 15,30,45 and 60 days after treatment.Xenografted scars were harvested for histologic analysis.The expression of CD34 and α-smooth muscle actin(α-SMA) were detected by immunohistochemistry.Cell apoptosis was detected by TUNEL method. Results 60 days after the treatment,there was no significant morphology change in TAC group.The scar was shrunk,soft and flat with lighter color in the microneedle and injection groups.The values of CD34 expression,α-smooth muscle actin and cell apoptosis in TAC group were 15.83±1.84,47.36±1.95,and 21.50±3.62,respectively,which had no significant difference compared with those after 15 days treatment(P>0.05).The decrease of the expression of CD34,α-SMA(P<0.05)and the increase of apoptotic cells(P<0.05)were observed 30 days after the microneedle treatment and 15 days after intralesional injection.60 days after the administration,the value of CD34 expression,α-SMA,and cell apoptosis in those two groups were 7.83±0.75,9.87±1.96,26.30±8.48 and 27.23±4.68,34.00±1.55,38.67±3.98,respectively,which had significant difference compared to TAC group(P<0.05).There was no significant difference (P>0.05) between the microneedle group and the injection group in the expression of CD34,α-smooth muscle actin,and cell apoptosis. Conclusion The anti-scar effect of triamcinolone acetonide delivered by microneedle roller was similar to that by intralesional injection in the nude mouse model.
